A proforma invoice is a document that accompanies every shipment of goods going outside the EU if it is a free exchange of goods, e.g. if you are sending samples.

Send the completed and signed form to praha@general-overnight.cz together with the extract from the Commercial Register (not older than 30 days), the VAT registration certificate and the EORI number certificate. For each subsequent shipment, we only need the invoice for the shipment and the exact description of the goods, including the Combined Nomenclature code.

For shipments to the US that contain food, beverages, vitamin or dietary supplements, or chewing gum. Please fill out this form, print it out and then give it to our courier with your shipment.

You must complete this declaration for all classic and video films, including video discs, blank tapes or films regardless of value. Please complete the declaration, print it out and give it signed to our courier with your shipment.

As the shipper, you are responsible for the proper documentation, labeling and packaging of the shipment containing dangerous goods.

Check the manufacturer's safety data sheet to make sure it is not an unsafe shipment. If so, fill in the declaration. This may avoid unnecessary delays or stoppage of the shipment.
